Make-Up Day at Clanzel T. Brown for Second Dose of Vaccine

A make-up day will be offered to individuals who received their first dose of the COVID-19 vaccine at Clanzel T. Brown Senior Center and missed their appointment for the second shot. Individuals must return to the site Monday, March 1, between 9 a.m. and 5 p.m.; an appointment is not necessary.
Individuals should have photo identification that includes their birthdate and the shot record card that was provided at their first visit. While copies of the Department of Health Vaccine Screening and Consent Form will be available at the vaccination sites, individuals are encouraged to bring a completed form to reduce their wait time; the form can be accessed at
