The Veterans Council of Duval County meets every third Tuesday of the month at American Legion Post 137 (5443 San Juan Ave., Jacksonville, FL 32210) at 7:00 P.M.

VyStar Memorial Trust Fund Application

The Veterans Council of Duval County is privileged that through the Naming Rights Agreement between VyStar Credit Union and the City of Jacksonville, it is able to give back to Veteran’s Service Organizations located in Jacksonville, Duval County, Florida that serve Veterans’ needs who reside in Jacksonville, Duval County, Florida. These funds are to aid our local veterans in an effort to improve their lives and moral. Grants will be awarded in a range of $500.00 to $10,000.00.

A Solicitation of Contributions Registration application is required to apply for the Vystar Memorial Trust Fund.

The IRS Federal Employee Identification Number (EIN) is a 9 digit number (format XX-XXXXXXX), do not fill with Florida State tax exemption number.
Nonprofit Status:

Has your organization qualified as a nonprofit, tax deductible entity, under the United States Internal Revenue Code 501(C)? If “NO”, please enter application date and provide a copy of your organization’s IRS form 1023.
The Contact name should be the person with the VSO that is responsible for decisions, able to gather information and understands the application and grant request. This is not just the person uploading the documents or filing out the application for the VSO.
Minimum amount to be requested shall be $500.00 and total amount requested should not exceed $10,000.00.

In addition to providing the information in this form, please include with your request the following attachments. Failure to include the listed attachments will result in your application being declined and having to restart this process.
Please upload a document which describes in 1,000 words or less a clear and concise statement of how these funds will be used. You must provide the impact and outcomes of the project to include anticipated completion dates.
I hereby certify that the aforementioned and enclosed information is complete and accurate, and that the organization is in full compliance with all applicable laws, statutes, ordinances, rules and regulations of any applicable governmental authority, including laws prohibiting support of terrorism.

If selected to receive grant funds, payment will be subject to final determination by IRS of 501(c) status, availability of funds, approval by the executive board, and approval by the City of Jacksonville
